First Visit
Your first visit in our office is a step in the right direction- toward better health! For your convenience, we’ve provided all the forms you will need to get started on your first visit in the link below. If you do not have access to a printer, we will provide a welcome packet for you at the office when you first arrive. Once the forms are completed, Dr. Carly will take you on a quick tour, and then begin your consultation to discuss in more detail your health history and wellness goals.
Next, we provide a comprehensive physical examination, where we evaluate your range of motion, analyze your posture, perform area-specific orthopedic and/or neurological exams, and evaluate the spine and associated musculature. Dr. Carly will fully explain each step in the examination process, but please don’t hesitate to ask questions! Depending on the results of your examination, x-rays may be required.
On future visits, you will learn the findings of your examination, review your x-rays, if taken, and a care plan will be instituted so you can begin your chiropractic care!
